The Health Information Department is responsible for the security of the electronic and paper medical records. Health Information is also responsible for overseeing the timely processing, completeness, and when necessary, the retrieval of all patient medical records. Health Information has an eight member team, most of whom are cross trained with multiple certifications. Team Leaders Margaret “Maggie” Jarreau, RN, Health Information Manager, and Dana Beauvais, MA, CCA, Assistant Manager, are very proud of the staff's accomplishments, dedication to service, and professionalism.

Health Information staff is available Monday - Friday, 8:00 AM until 4:30 PM. After hours, the Health Information Manager or designee is available by telephone. The Hospital Charge Nurse or designee may access the file room to obtain records after regular office hours for the physician.

The scope of services provided by the Health Information Management Department is based upon a collaboration and information sharing format to enhance patient care.

Health Information services include the provision of an accurate patient record, with emphasis on the following:

  • Record processing

  • Record abstracting, analysis and coding

  • Transcription of dictation for the medical record

  • Correspondence

  • Record retrieval, filing and storage

  • Birth certificate function

If you wish to obtain a copy of your medical record, the proper protocol for release of the medical record is as follows:

  1. Report to Health Information located on the third floor of the hospital during office hours.

  2. You will be required to complete a “Release of Information” form.

  3. Two identifiers are required prior to release of your medical record. One identifier must be a photo identification which can be in the form of a driver's license, student I.D., or passport. The Health Information staff will ask questions regarding your birthdate, the date of service for the records requested, or your social security number to satisfy the second identifier requirement.
